By joining the Brightwater Welcome Team, you will use your clinical knowledge and background to guide new clients and their families through the initial assessment process to ensure their care and support requirements are met, whilst making the process as easy and comfortable as possible.

With a focus on Residential Aged Care clients, you will perform comprehensive assessments from a clinical, pre-planning, and financial perspective, and provide information on all relevant Brightwater services to encourage informed client decisions and generate business growth opportunities. 

This role is being recruited on a full-time, 12 month, fixed-term contract, however, we would consider part-time (0.8 FTE) applications. You will initially be based in Currambine until mid-2022 when we will be relocating to our new, state-of-the-art building in Inglewood.

Key responsibilities include:

Perform comprehensive client assessments, ensuring relevant information/factors (including the level of support and predicted level of funding) are appropriately identified and captured for consideration by the client, in line with Brightwater’s Clinical service offering.
Proactively assist clients in navigating through the assessment process, including working collaboratively with others to ensure all client care opportunities are identified and developed with the client.
Seek out and act upon business growth opportunities to present, promote and sell Brightwater services to generate business growth.
Maintain all assessment documentation; hard copy and electronic copy, as required, ensuring accuracy and integrity of all information.

To be successful in this role you will have:

Relevant tertiary qualification in Nursing, with current registration with AHPRA
Experience with assessing client care requirements
Health industry case management experience; with the ability to effectively manage new clients
Exceptional customer service and selling skills
The ability to organise and prioritise tasks, manage a busy appointment schedule, and maintain current and accurate documentation
High level of verbal and written communication skills, including preparing reports against identified targets and Key Performance Indicators (KPIs)
Demonstrated ability to build and develop effective relationships across a range of stakeholders, both internal and external to the organisation
Sound understanding of the aged care, disability services, community care support sectors, and the health industry, including current knowledge of funding and relevant changes impacting all sectors
Previous experience in eFrontoffice, Carelink+, highly desirable
